Medication Forms and Action Plans For Asthma, Seizures, Food Allergies

If your student requires medication during the school day, please have your doctor complete the appropriate forms that can all be located here.  Please remember, all medication must be accompanied by a doctor's order and must be in the original prescription packaging.  All forms are valid for ONE school year and must be completed and signed in their entirety.  Thank you for your cooperation.
